Important Information to Consider Prior to Booking an Appointment

Medication Management and Therapy: Our practice offers medication management and therapy services to support your mental health journey.

Crisis Management: Please be aware that we do not provide crisis management services. In case of an emergency or a life-threatening situation, whether during our regular business hours or outside of them, please dial 911 immediately or visit the nearest emergency department.

Appointments:  We kindly ask for a minimum of 24-hour notice for appointment cancellations or rescheduling. This allows us to offer the appointment slot to someone on our waitlist, ensuring efficient care for all patients.

Cancellation or No-Show Fee: Patients who cancel an appointment or fail to attend with less than 24 hours’ notice may be subject to a $100/$150 cancellation or no-show fee. Please note that you are financially responsible for appointments not rescheduled or canceled with appropriate advance notice.  The fee is $100 for established patients and $150 for new patients.

Late Arrival Policy: Arriving more than 10 minutes late for an appointment may result in cancellation or rescheduling.

Emergency Circumstances: If emergencies require changes to your appointment, please contact our office promptly. While we will make every effort to accommodate your needs, immediate rescheduling of missed appointments cannot be guaranteed.

Missed Appointments: Repeated missed appointments without prior notification may result in discharge from our practice. This policy ensures that we can effectively provide care to all patients.

FMLA and Disability Psychiatric Evaluations for Employment and Disability: It’s important to understand that we do not offer single psychiatric evaluations for purposes like employment, medical clearance, disability claims, or legal issues. Nevertheless, for our existing clients, we may consider these evaluations on a case-by-case basis.

If you need our provider’s help with completing STD/FMLA forms, please schedule an appointment to address this matter.
